Programs/projects that directly and positively change health habits, behaviors and/or lifestyles through prevention, intervention and education strategies for residents in Nemaha, Riley, Pottawatomie, Jackson, Jefferson, Douglas, Shawnee, Wabaunsee, Osage and Lyon Counties.
